今年11月,台北馬偕醫院急診人數再創新高,達到10萬8千764人,台大、三總也同樣居高不下。各界關心急診問題之際,《報導者》推出《急診人生》新聞遊戲,這是一款富含真實情境的急診室經營創新夢幻之作,為台灣第一個急診室新聞遊戲。(新聞遊戲是什麼?請見 「為什麼用遊戲做新聞」 )

遊戲背景鎖定在台灣的一間大型醫學中心,面對健保體制的崩壞、沒膽改革的政府以及愛跑大醫院看病的人民,擁有拯救急診室命運能力的鍵盤醫師,將在一次又一次的真實的醫療突發狀況中突圍,試圖拯救病患的生命。你,將在人類的極限體力、醫生的使命和病患的生命中作出抉擇,準備好了嗎?

Q:為什麼我一定要先救治紅色重症病患?先解決綠色的輕症病患不是比較快嗎?

吳醫師:遊戲中的「紅黃綠」點擊規則,是真實世界中的急診室救治原則。在急診室裡,不是你先來就可以先看,醫生會根據每個病患的情況做「檢傷分類」,將病患們分為5個級數,嚴重到輕微,從1到5級區分,如此才能適當運用醫療資源來診治病患。

Q:欸你們那個遊戲設定很誇張,怎麼有人因為香港腳送急診?

吳醫師:客官冤枉啊,這是千真萬確的案例啊。除了香港腳,媽媽因為孩子被蚊子叮咬,叫救護車送急診、 OL因為不想排門診,直接殺進急診室要求醫生幫他擠痘痘也都是真實案例。想了解這個誇張的遊戲設定背後原因,請洽後續報導。

Q:也太多留觀病患?我哪裡處理的完啊!(摔手機)

吳醫師:這也是真實劇情之一,也是急診室壅塞最大原因:病人滯留時間長。各位大大可能覺得很奇怪,急診室滿床了不就送上去醫院病房就好了嗎,為什麼要卡在這裡。但其實背後隱含很多原因,包含轉診調度、病人的情況、各醫院病床制度等。吳醫師在這裡無法一一解釋,要麻煩報導者幫我解釋了。要成功破解這個討厭的遊戲難關,唯有大家一起努力啊。

Q:我已經很盡力安撫病患了,但老是出現那位揮大刀的大哥害我鎖屏!

吳醫師:急診室是整個醫院的第一線,風險較其他專科都還要高,也較常需要處理突發事件。急診室沒辦法拒絕病人,在病人量越來越多的情況下,醫師可能同時就要照顧好幾個病患。因此當病患或家屬情緒不穩,醫病關係處於緊繃的情形,急診暴力的情形可能就會發生。吳醫師來講個秘訣:來急診室看病,多體諒沒事,沒事多體諒。

Q:光是病患就處理不完了,我根本沒時間去點擊醫生的生理需求啊。

吳醫師:急診醫師是輪班制,一個班12個小時,每個醫院排班時間不一樣。下班之前還要先交班給另外一個醫師,讓他了解留觀病患狀況。人數如果太多,交班有時候會超過半小時。以北部某醫學中心為例,一個月大約有9000到10000人次的病人,每天等床留觀的病人就可能高達到70到80床。一般來說,白天班會比晚上忙碌,醫生有時候甚至連坐下來、吃飯的時間都沒有,就這樣到隔天的交班。

Last November, the number of emergency room patients in Taipei Mackay Memorial Hospital hit a new record high, reaching108,764. The Reporter deliver the first emergency room news game in Taiwan to help the public recognize the difficulties and problems of the emergency room through innovative digital storytelling.

As a player, you’ll face the decay of the healthcare system, the government’s inability to reform the medical system, and the public’s waste of medical resources. The players have to fix accidental situations to save patients’ lives.

1.Authentic and profound scenario:We had interviewed numerous emergency room doctors to learn the actual environment in the emergency room and design the news game based on true context.

2.Adorable and charming design style:All the designs of the characters and scenes in the news game are based on the real circumstances in the emergency room. The actions, facial expressions, and the mysterious characters that appear randomly are designed with sophisticated details and adorable style.

3.Vintage game playing:“The Daily Life of Emergency Room” is simple and intuitive for all the players. The news game includes emergent scenarios to let you experience the real ambiance in the emergency room.

FAQ

Q: Why do I have to treat the most serious patients first? Is it faster to treat the less serious patients?

Dr. Wu: In the news game, patients bodies will appear in different colors according to their injured level. Red will be the most serious, then the yellow and green. In reality, doctors will perform the triage to sort out patients into five levels based on the severity of their condition and determine the priority of patients’ treatments.

Q: The setting of this news game looks ridiculous. How can a person go to the emergency room because of Athlete’s foot?

Dr. Wu: It’s totally true. Athlete’s foot is one of the absurd cases. A mother called an ambulance because her children was bitten by a mosquito. An office lady went to the emergency room and asked a doctor to help her squeeze pimples because she didn’t want to make an appointment. Q: There are too many patients that have to be hospitalized after the treatment. How can I treat all of them?

Dr. Wu: It’s exactly true, too. The main reason that causes the emergency room becomes overcrowded is because patients stay too long. Why don’t we move those patients to general ward after the treatment? There are many reasons, including referral arrangement, patients’ conditions, and hospital bed system, etc.

Q: I’ve tried hard to soothe patients, but I can’t prevent violence.

Dr. Wu: Doctors in the emergency room always afford higher risks than other department, and they have to handle sudden incidents. They can’t refuse patients and have to treat many patients at the same time. If patients or their family members feel upset or irritable, physician-patient relationship will become more and more tense and violent incidents will happen possibly.

Q: I have no time to take care of doctor’s physical needs.

Dr. Wu: Doctors in the emergency room work in shifts, and one shift lasts twelve hours. Before you are off the duty, you have to hand over the work to another doctor and help him realize hospitalized patients’ conditions. If there are too many patients, the job handover will last more than thirty minutes. Take a teaching hospital in the northern area for example, they usually receive 9,000~10,000 patients in a month, and the number of patients that have to be hospitalized amounts 70~80 beds every day. In general, the workload of day shift is heavier than night shift. Sometimes doctors have no time for resting or eating.
